Here's how recurring payments work:

You authorize regularly scheduled charges to your credit card. You will be charged the amount indicated below each billing period. An email reminder for the payment will be sent to you two days prior to each payment and you will be emailed a receipt for each payment.  You agree that no prior-notification will be provided unless the date or amount changes, in which case you will receive notice from us prior to the payment being collected. 

I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify Big Country to any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. If the above noted payment dates fall on a weekend or holiday, I understand that the payment may be executed on the next billing day. I certify that I am an authorized user of this credit card; so long as the transactions correspond to the terms indicated in this authorization form

I hereby acknowledge and understand that martial arts is a physical activity and learning self-defence skills involves some physical contact. Big Country and/or its staff and students are not to be held liable or responsible for any risks and/or injuries as a result of training.

1. Recognize and understand that martial arts training is a physical contact activity and that my participation might result in serious injury.
2. Recognize and understand that such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the regulations of participation, or the conditions of the premises, or of any of the equipment used.
3. Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of training and competition.
4. I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical and mental well-being during intense training practice and/or competition.
5. Grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide me with medical assistance or treatment for such injury.
6. Release, waive, discharge and covenant not to sue, Big Country, its affiliated organisations, instructors and personnel, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
7. I understand that when I choose the recurring membership option that my credit card will be automatically charged on the same date of every month with an email reminder two days prior to the payment withdrawal.
8. I grant permission for Big Country and any photography team to take photos of myself and or child during class times to be used for social media advertisement. I understand that if at any time during class times that I do not want mine or my child/children's picture taken or used, I have the right to inform Big Country of my decision and that it will be respected.
9. I understand that there are no refunds on Big Country memberships and that memberships are due on the date provided.
